To receive a document, you need:
1. Copy of passport (international or internal)
1.1 For the international passport a copy of the first page is required.
1.2 For internal passport the copy of pages 1-2, 3-4, 10-11 is required.
1.3 Another identity card is required in case of expiration of documents of the previous current citizenship.
1.4 Another identity card is required in case of acquisition of citizenship of the country in which you currently reside.
